The 12 Required forms for EPA Personnel Files
- AA-7, a muliple layer form, not available online but can be picked up from Dept, Dean or Provost office.
- AA-14, a muliple layer form, not available online but can be picked up from Dept, Dean or Provost office.
- Official Transcript, comes from candidate.
- Vita, comes from candidate
- Conflict of Interest (Not online, pick up from Dept or Dean's office)
- Contract/Appointment Letter, created in Dean's office when position approved, and kept in dept office until candidate's name is known.
- EPA Application, available online, Western Carolina University EPA Application (PDF Format)

- W-4 (taxes take-out) Federal / State Tax Form (W-4 / NC-4)

AA-7, AA-14, I-9, AA-21, Veteran Status, EEO Data Form are in Appendixes of this pdf file Hiring Guide for Faculty and Non-Faculty Administrative and aa7 and aa14 are in duplicate so cannot be used from printouts. Pick up instead.

Note that I-9's MUST be completed within 3 days of employment.
The AA-7 , AA-14 and AA-21 with attached Vita will be sent directly to Irene Welch, Office of the Provost.
Part-time Faculty and EPA Non-Faculty paperwork is collected by departments and forwarded direcly to Donna Watson, Human Resources. Please make sure all forms are completed before sending.

Full-time Faculty will be mailed contract and all forms listed above from Office of the Provost. Completed paperwork is to be forwarded to Human Resources.
